As the Department of Defense weighs the future of TRICARE’s Autism Care Demonstration (ACD), military families, healthcare providers, and policymakers are engaged in an increasingly important discussion: Should applied behavior analysis (ABA) therapy become a permanent covered benefit, or should it continue as part of a demonstration program designed to evaluate autism care?
The answer could shape how thousands of military-connected children with autism spectrum disorder (ASD) access care for years to come.
A Demonstration Program With Lasting Impact
Established in 2014, the Autism Care Demonstration was created to evaluate the effectiveness, quality, and cost of providing comprehensive autism services—including ABA therapy—to eligible TRICARE beneficiaries. While demonstration programs are typically intended to operate for a limited period, the ACD has been extended multiple times as the Department of Defense continues to assess clinical outcomes and program performance.
Today, the demonstration serves nearly 17,000 TRICARE beneficiaries diagnosed with autism spectrum disorder, making it one of the Military Health System’s largest specialized care initiatives.
For many military families, however, the program’s temporary status has created ongoing uncertainty. Families must navigate changing program requirements while balancing the realities of military life, including frequent relocations, deployments, and disruptions to established healthcare networks.
More Than a Debate About ABA
Applied behavior analysis is one of the most widely used interventions for children with autism, focusing on improving communication, social interaction, adaptive skills, and behavior through individualized treatment plans.
While ABA is supported by many clinicians and families as an effective intervention, it has also been the subject of discussion within the broader autism community. Some autistic self-advocates have raised concerns about aspects of traditional ABA practices, while practitioners note that contemporary ABA increasingly emphasizes individualized goals, patient assent, and quality-of-life outcomes.
As discussions continue over whether ABA should become a permanent TRICARE benefit, some stakeholders argue that the conversation should encompass the broader delivery of autism care rather than focusing solely on a single therapy.
Jim Maguire, co-founder of GMS, believes the larger issue extends beyond the future of the demonstration itself.
“The question isn’t simply whether Applied Behavior Analysis should remain a demonstration or become a permanent TRICARE benefit,” Maguire said. “The larger question is whether the Military Health System is delivering care that is evidence-based, accessible, coordinated, and producing meaningful long-term outcomes for children and their families.”
Measuring Success Beyond Coverage
Supporters of transitioning ABA into a permanent TRICARE benefit argue that autism is a lifelong neurodevelopmental condition and that medically necessary treatment should be supported through a stable, permanent benefit structure rather than an ongoing demonstration program.
Others emphasize that the Department of Defense should continue evaluating both clinical outcomes and fiscal sustainability before making long-term policy decisions.
Maguire said the existing program has already demonstrated both its reach and its importance to military families.
“Every military family deserves timely access to clinically appropriate care for their child, regardless of where they are stationed,” he said. “The Autism Care Demonstration currently serves nearly 17,000 TRICARE beneficiaries with autism spectrum disorder and represents a significant investment in supporting military families.”
He added that early program findings point to encouraging results while highlighting the importance of family involvement in treatment.
“Reported outcomes to date suggest many participants are making meaningful clinical progress, while also underscoring the critical role that parents and caregivers play in achieving successful outcomes.”
Balancing Access, Outcomes, and Readiness
Healthcare providers have long pointed to provider shortages and continuity-of-care challenges within the military health system. Frequent permanent change of station (PCS) moves can interrupt treatment, forcing families to establish care with new providers in unfamiliar communities.
Advocates argue that a permanent benefit could encourage greater provider participation in the TRICARE network, helping improve access and consistency of care. At the same time, Department of Defense officials continue evaluating the demonstration’s long-term costs, clinical effectiveness, and quality measures before determining the program’s future.
For Maguire, those factors should all remain part of the decision-making process.
“As policymakers consider the program’s future, the decision should be guided by longitudinal clinical outcomes, beneficiary access, fiscal stewardship, and the recognition that autism care extends beyond ABA therapy alone.”
Looking Ahead
No final decision has been announced regarding whether ABA therapy will transition into a permanent TRICARE benefit or whether the Autism Care Demonstration will continue in its current form.
Regardless of the policy outcome, military families say consistency in care remains essential for children with autism, whose developmental progress often depends on uninterrupted access to qualified providers and coordinated services.
Maguire said that should remain the benchmark for evaluating the program’s future.
“Military families already navigate frequent moves, deployments, and disruptions. Their healthcare should provide consistency, not additional uncertainty.”
Ultimately, he said, the measure of success is straightforward.
“Whatever path DoD ultimately chooses, success should be measured by whether children receive the right care at the right time, families can access qualified providers, and the benefit strengthens both family well-being and military readiness.”
